We are a mental health agency that provides services for people with intellectual and developmental disabilities. We are looking staff in the Wake Forest area. The following is expected of each potential candidate:

• Establishes a caring therapeutic relationship with clients. Provides appropriate habilitative and personal care while ensuring that clients’ safety, health and well-being are maintained.
• Correctly implements Individual Support Plan/Plan of Care including participating with the client’s during activities in the community as defined in the Individual Support Plan.
• Demonstrates an understanding of client rights and confidentiality and ensures these rights are protected.
• Demonstrates an understanding of communication techniques/strategies to support the client and the person-centered approach.
• Participates in initial and ongoing Individual Support Plan development.
• Provides transportation for clients as defined in the Individual Support Plan/Plan of Care.
• Provides appropriate level of supervision as outlined in the Individual Support Plan.
• Demonstrates knowledge of TCHS Policies and Procedures defined in the employee handbook for direct care staff.
• Completes documentation as required. Records accurate, informative, and legible notes which reflect applicable goals.
