PACE Referral and Communications Coordinator (Bilingual English/Spanish)

Monday-Friday 8am-5pm

The PACE Referral and Appointment Coordinator is responsible for scheduling external and internal appointments in collaboration with participant/caregiver, taking participant preferences into account. The Coordinator will schedule all transportation related to appointments. The Coordinator will manage the referral work queue for external appointments, scheduling those appointments at the appropriate location, and entering the appointments in EPIC; The Referral and Appointment Coordinator will also collaborate with the secretaries from the PACE sites to ensure good coordination between PACE Center, IDT, Participant/Caregiver and external vendors; The Referral and Appointment Coordinator will work closely with transportation scheduler for maximum efficient use of resources. Bilingual (English/Spanish) required.
Essential Duties & Responsibilities:
  • Maximize efficiency of transportation by coordinating appointments to same locations with participants across PACE centers.
  • Facilitates external appointments utilizing phone, EWS, SDK, and Epic.
  • Efficiently manages referral work queues in Epic for all external appointments.
  • Utilizing appointment reports, schedules all transportation for PACE Center appointments.
  • Establish and maintain professional collaborative relationships with internal and external customers.
  • Coordinates transportation in an efficient manner: Enters all data in Routematch, Registers new participants in Routematch, schedules standing order trips for PACE Center, Wellness Center, and recurring specialty appointments.
  • Processes all cancellations and changes of all appointments.
