The Team Member We Are Searching For
The ideal Occupational Therapist is passionate about delivering superior, patient-centered care and is an excellent communicator with all types of patients. Under the direction of the Regional Director of Rehabilitation Services, the Occupational Therapist provides and directs Occupational Therapy services to patients including assessment, treatment, program planning, and implementation, related documentation, and communication.
What you will need to accomplish
-
Evaluate patients, interpret evaluation findings to determine the nature and extent of dysfunction
-
Establish treatment goals and plans treatment to achieve established goals
-
Initiate discharge planning for occupational therapy
-
Delegate a portion of the treatment program to the appropriate supportive personnel
-
Coordinate the treatment program with other health care personnel as appropriate
-
Provide advice and education to patients and their family
-
Regularly assess clinical signs and symptoms to determine effectiveness of treatment, progress of patients towards goals, the need for modifying goals and treatment
-
Discharge patients from occupational therapy when the patient has received maximum benefit from occupational therapy.

Provides accurate, timely documentation of occupational therapy patient management

Demonstrates progress toward meeting the criteria for professional practice

Perform related duties as assigned
What do we require from you?
Minimum Qualifications
-
Current driver’s license, vehicle insurance, safe driving record, and reliable transportation are required
Education:
-
Valid California OT license required
Experience:
-
One (1) or more years of experience as an OT in home health preferred
-
Experience in Home Care HomeBase EMR system a plus
Skills and Abilities
-
Excellent communications, both written and verbal, organizational, presentation and facilitation skills
-
Ability to speak, read, and write English
-
Bilingual (English/Spanish) a plus
-
Self-directed with the ability to work with little supervision
-
Excellent observation skills
-
Flexible and cooperative in fulfilling role obligation
-
Maintains confidentiality in all aspects of the job

Respects the confidentiality of information in patient and employee records

Proficient in Microsoft Office
-
Schedule flexibility is necessary to meet the needs of patients as well as the leadership

Ability to use accurate, clear, and concise oral and written communication
