Position
Southern Tier Medical Care is searching for a full-time Nurse Practitioner or Physician’s Assistant to provide acute and/or problem focused care to Skilled Nursing facility patients under the general supervision of a Primary Care Physician. Under the direction of the physician, the NP/PA will provide medical management, patient education, and care coordination to patients requiring care for chronic care needs.
Experience Requirements:
Minimum 1 year as a Nurse Practitioner/Physician’s Assistant level experience in a SNF setting. 2-3years experience as a NP/PA preferred.
Educational Requirements:
Graduate of an accredited Nurse Practitioner/Physician’s Assistant program. BSN degree required. MSN degree preferred.
Licensure/Certification Requirements:
State license with NYOP.
Hours of Work: Wed & Fri, 8:00am – 5:00pm
Job Type: Full-time
Pay: $110,000.00 - $140,000.00 per year
