We are seeking an Advanced Practice Provider to join one of the nation’s most integrated healthcare systems, with over 1,250 providers in Indianapolis, IN. This onsite, Employer Health opportunity with Hamilton Southeastern Schools (HSE) is located at our Saxony Pavilion 13121 Olio Road, Suite 200. Fishers, IN 46037.
Highlights
Community Health Network has an excellent opportunity for a family medicine trained advanced practice provider to work at our onsite HSE clinic in Fishers, Indiana.
- Single, robust electronic medical record system (EPIC)

- Practice hours Monday – Friday 7:00 am – 6:00 pm; Saturday 8:00 AM – 12:00 PM
- Opportunity to practice autonomously
Education
Educational background consistent with requirements for licensure and certification with the State of Indiana. Provider must be family medicine trained and family medicine board certified.
Skills
Demonstrate a high level of expert skill in delivering exceptional, quality care while enhancing the well-being of the patient. Minimum two to three years of experience as a provider is required.
