Locations hiring for: Laveen, Goodyear, Phoenix (north and south), Mesa, Peoria, Surprise, Maricopa City, Casa Grande, Gilbert, Tempe, Chandler, Queen Creek, Glendale.

We are looking for independently licensed social workers (LCSW), independently licensed professionals (LPC), and licensed independent substance abuse counselors (LISAC), or Associate level licensed professionals (LAC/LMSW), or Masters level counselors and social workers.

Teri’s Health Services is hiring Mobile Clinicians, Licensed/Masters level unlicensed counselors and social workers to be mobile in residential treatment settings, school districts as behavioral health school counselors, group homes and nursing care homes and assisted living facility clinicians. We work as a team and individually. Those who are interested need to have a strong sense of working with much autonomy, ability to keep organized and manage their time, and flexible in their work with clients as these settings are open sites with people enrolling frequently and intakes are ongoing. Assessment, treatment planning, and all areas of clinical work clinicians will learn the value of and how to do.

· Travel to 1-3 sites per day, most sites are one site location per day only

· Clinicians are hired to work at sites closest to their own residence so driving is reduced and not a great number of hours spent in the car.

· Conducts initial assessments to document history and identify preliminary issues for treatment focus by interviewing the patient, their family members and outpatient team.

· Engages in daily groups, individual, and case management services

· Participates in the development and implementation of treatment plans and updates treatment plans as required.

· Provides individual, group, couples, and family process-oriented therapies using various professional treatment modalities.

· Demonstrates the ability to documents in client's chart in a way that is professional and appropriate.

· Documents patient care within 24 hour

· Reports incidents of suspected abuse using prescribed forms and protocols from the relevant community/state agency.

· Responsible for utilization management for assigned patients and assists in the assessment and solution of utilization review problems.

· Maintains working relationships internal and external sources in ART/CFT meetings

· Maintains up to date training as internally is allotted

· Committed to THS mission and vision

· Attends supervision as well as clinical oversight and uses supervision time appropriately.

Requirements

· 2 years of experience preferred

· Licensed or license experience preferred

Masters degree in counseling, marriage and family therapy, substance abuse counseling, or social work, license is preferred not required.

· Must be 21 years of age, have a current negative TB test or medical clearance and meet qualifications for fingerprint clearance. CPR, First Aid, CPI (or obtained within 30 days of hire). Valid drivers license and vehicle insurance. Vehicle must be in working order. Maintain vehicle insurance and vehicle registration.

Job Types: Full-time, Part-time

Pay: $55,000.00 - $90,000.00 per year
