Oakleigh of Macomb is looking for a Memory Care Coordinator!
The Memory Care Coordinator is responsible for providing overall leadership and management of the reminiscence neighborhood. Summary of responsibilities includes promoting excellence in resident care and programming, providing outreach and support to families, and being resourceful and knowledgeable with the Family Resource Library and the evolving information on Alzheimer's disease. Responsible for championing the designated care manager concept throughout the neighborhood, assisting in the recruiting, hiring, and training of team members; completing staffing, scheduling and timekeeping review, team member recognition, engagement, and performance management.
EXPERIENCE & QUALIFICATIONS
- One year experience preferred in assisted living, long-term care, and experience with memory-impaired senior population.
- One year of supervisory and management experience including hiring staff, coaching, performance management daily operations supervision, discipline, and counseling.
- Demonstration of proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel).
- Ability to work weekends, evenings, and flexible hours, available for our customers at peak service delivery days and times.
Essential Duties:
- Plans, organizes, develops, and leads the overall operation of the reminiscence neighborhood(s) In accordance with federal, state, and local laws, and company standards, guidelines, and regulations.
- Designs, schedules and facilitates the reminiscence program incorporating Life Skills and focusing on creating pleasant days; including a variety of dementia-appropriate activities.
- Prior to move‑in review the resident’s file and schedule the Family Meeting to discuss the Resident’s Profile; coordinate the health care needs with the health care coordinator; ensure the resident has a current Service Plan.
- Facilitates the residents’ Service Plan and Daily Assignment Sheet development and monitors ongoing progress and practices to ensure completion of tasks including hydration and resident engagement In Life Skills programming.
- Understand the recognition of resident changes in condition and takes appropriate action.
- Establishes a cooperative relationship with the local Alzheimer's Association chapter or other Alzheimer's advisory organization.
- Facilitates network contacts and arranges for speakers from community groups/organizations to provide educational support for families of the residents; serves as the liaison with the community Alzheimer's specialists.
- Ensures monthly family support group meetings are planned and facilitated.
- Understands the internal cost associated with all resident care programs.
