Job description
Summary:Manages and maintains the integrity of the home care patient medical records to include tracking and timeliness of physician orders to meet regulatory requirements. Runs daily physician order runs for mailing, filing of paper forms into hard copy record, scanning electronic documents into the lifetime electronic records, and maintains the patient record room. Audits new admission records for presence of required forms, assembles blank admission folders, identifies & prepares medical records for quarterly record review, payer & attorney audits, and government agency review. Provides back-up coverage of the reception desk in the absence of the receptionist and back-up for the schedulers. Reports to the WCHC Clinical Director.
Responsibilities:
1. Maintains the integrity of the home care patient medical records to include tracking and timeliness of physician orders to meet regulatory requirements, running daily physician order runs for mailing/faxing, filing of paper forms into hard copy record, scanning electronic documents & wound photos into the lifetime electronic records, auditing new admission records for presence of required forms, and assembling blank admission folders. 30%
2. Maintains the patient record room where the hard copy records are stored, assures that patient records are filed in the correct filing draws, assures that each new patient has a hard copy medical record, moves discharged patient records out of the active files, and boxes prior years' discharged records for storage. 30%
3. Maintains confidentiality and safety of patient medical records and information. 5%
4. Stocks blank patient documentation forms, copy paper supplies, and maintains shredding bins. 5%
5. Provides back-up coverage to reception desk and scheduler in the absence of these personnel. 15%
6. Identification & preparation of medical records for quarterly record review, payer & attorney audits, and government agency reviews upon request. 15%
7. Fulfills all compliance responsibilities related to the position.
8. Performs other duties as assigned.
Other information:
Required: Command of the English language, fluent in reading & writing the English language, use of computer software programs such as Word, Excel, & Outlook, experince working with electronic medical records, scanning devices, faxes, and phone systems. Excellent communication skills, organizational skills and prioritization skills. High School diploma mandatory.
Minimum Experience: one year
Desired: Associate Degree preferred. Proficient in Excel, medical assistant training/certificate, prior experience working in a medical office.
