Job Summary:
The duties and responsibilities of the MDS/Care Plan Coordinator include completing MDS’s to ensure all state and federal guidelines are met.
Essential Functions:
Ensure care plan meetings are scheduled.
Schedule MDS’s & open them at the beginning of each week.
Close, lock & transmit to state website weekly and as needed to ensure no late submissions.
Update quarterly assessments.
Rotate Call.
Update the THMP forms.
Participates in ongoing training and in-services for improving knowledge and updating skills.
Performs other assigned tasks as requested and in accordance with policies and procedures.
Establish and encourage an atmosphere of optimism, warmth, and interest in residents’ activity preferences and needs.
