Here is an opportunity for a Licensed Practical Nurse (LPN) to join Blessings4Ever Home Care as a Nurse Coordinator. As a Nurse Coordinator, you will conduct initial client assessments and provide support, training and mentoring for agency Care Coordinators. The ideal candidate is an LPN with prior Home Care or similar experience. The Nurse Coordinator will identify consumers at-risk for hospital readmissions and makes recommendations for population health management. Population Health experience is a plus.
