LPN – ALTIMATE CARE

Altimate Care is hiring! Are you an overworked LPN that is tired of running the halls and being in short-staffed facilities? Our LPN will provide assistance with daily living tasks and personal care services to clients in their homes in accordance with an established care plan.

Altimate Care provides home health services to our patients by delivering one on one nursing care in our patients’ homes. This helps promote healing, reduces stress and allows the client to remain in their home. We’re seeking a Licensed Practical Nurse to join our team!

Responsibilities

• Monitor vitals and perform skilled visits
• Completes appropriate visit records timely and accurately
• Perform patient care using a team approach with your RN Case Manager
• Works with family members to instruct on disease management as needed
• Communicates changes in the patient's condition and/or needs timely and accurately to care team

Qualifications

• Ability to participate in physical activity, including but not limited to standing and lifting
• Ability to perform job for extended periods of time
• Organized and dependable
• Able to work independently
• Prior home health care experience is a plus (will train)
• Valid LPN license

Salary

$25 per visit plus travel allowance

Schedule

Monday to Friday 8:30am to 5pm
