POSITION TITLE: Licensed Social Worker

REPORTS TO: Mental health clinical supervisor (e.g. LISW, LPCC, psychologist, other independently licensed mental health professional) and Director of Community Services.

PERSONS SUPERVISED: QBHS staff for clinical oversight and supervision, Community Behavioral Health
Clinicians, others as directed

PAY RATE: Exempt, $50,000.00 - $60,000.00 per year commensurate with experience.

SCHEDULE: Monday – Friday; Schedule varies by work location

PURPOSE: The LSW provides assessment, individual service/treatment plan development, case management, and other mental and behavioral health services to clients with mental health illness within your scope of practice. This position would provide assessment, treatment planning, and case coordination for clients across multiple locations, such as school, work, and home, and across one or more services and funding sources including behavioral health and waiver. Must have strong case management and communication skills, as well as the ability to work effectively with families and treatment partners. Travel required as needed.

This position will receive 1:1 and/or group supervision from supervisors through direct observation, clinical meetings, and/or videos. Supervision is provided regularly and is documented. In addition, community behavioral health clinicians are required to attend team meetings, staff professional development sessions per their specific staff development plan, scheduled training, and continuing education as required by their licensing board. This position will be supervised by both a Licensed supervising social worker as well as the community manager or director. Provide initial training for new team members for the delivery of mental health sessions only.

This position requires the staff member to maintain all current licenses as required by the Ohio licensing board(s).

EDUCATION REQUIRED: Bachelor’s degree or higher

CREDENTIALS REQUIRED: Ohio license such as: LSW, LPC, MFT, LISW, LPCC, IMFT, Ph.D. Supervisory status preferred.

OTHER REQUIREMENTS (KNOWLEDGE AND SKILLS):

  • Advanced verbal and written language skills, and an ability to communicate effectively with clients, parents/guardians, and collateral sources (e.g., pediatricians, teachers)
  • Education and experience diagnosing and treating common psychiatric diagnoses is preferred, as is working knowledge of DSM-V and ICD-10
  • Knowledge of autism spectrum disorders including common evidence-based treatments such as cognitive-behavioral therapy.
  • Knowledge of basic computer skills (Microsoft Word, Excel), as well as skills in internet navigation, email management, and compliance with HIPAA rules
  • Experience with the use of an electronic health record is preferred
  • Ability to adapt to and respond appropriately to acute events, such as using good clinical judgment during risk assessments and mandated reporting.
  • Excellent interpersonal skills in order to develop positive relationships with clients, families, peers and community representatives
  • Demonstrated punctuality, timely completion of documentation, and the ability to meet productivity demands are required
  • Ability to work collaboratively within a treatment team

SPECIFIC “JOB DUTIES” INCLUDE BUT ARE NOT LIMITED TO:

  • Complete a comprehensive assessment with clients and evaluate the psychological and social problems and needs following current regulations and designated policy and procedure.
  • Develop individualized treatment plans and/or individualized service plans (ITP/ISP) for each client following current regulations and designated policy and procedure in conjunction with our professionals part of the team such as BCBA, SLP, OT etc.
  • * Provide CPST/TBS services by implementing specific, measurable, and individualized services to each person served focusing on the individual's ability to succeed in the community; to identify and access needed services; and to show improvement in school, work and family and integration and contributions within the community following current regulations and designated policy and procedure.
  • Identify as the designated case manager for all of your client cases and assist in case management including but not limited to the coordination with healthcare providers/agencies for support in the client's home.
  • Provide complete and detailed documentation for treatment(s) provided to a client as required by law to include but not limited to: 1) progress or lack of progress toward treatment goals identified on the client’s

ISP/ITP, 2) the continuing need for services, 3) all other required fields (e.g. service date, client name, beginning and end time of service, etc.), and 4) your signature and designated mental health credentials.

  • Maintain a complete and adequate individual client record for each client following regulations and designated policy and procedure.
  • Help maintain and keep current a client resource binder to be available to all clients in the lobby.
  • Provide CPST, TBS, PSR, and other therapy clinical supervision as assigned.
  • Consistently demonstrate core competencies required for the position.
  • Attend supervision and administrative meetings as directed.
  • Meet productivity requirements.
  • Conduct training and supervision of new QBHS staff as directed.
  • If deemed necessary by the team and added to the service plan, provide counseling and therapy activities/psychotherapy to the client, the client’s family members, and others significant to the client as deemed medically necessary following the current regulations and designated policy and procedure
  • Conduct ongoing staff training if requested and presentations as directed on topics including but not limited to:
  • Cultural sensitivity and diversity,
  • Characteristics of the population served,
  • The community behavioral health system, social service systems, the criminal justice system, and other healthcare systems related to Autism Spectrum Disorders.
  • Complete training and education requirements specific to CPST/TBS services and their provision.
  • Participate in staff development, education and training. Training may be provided by direct supervision, attendance at conferences and workshops internal and external to Empower, online training, educational coursework, etc. Training shall include at minimum topics that maintain or increase competency, topics specific to the population served, and cultural competency in your provision of service.
  • Receive and work towards goals developed in collaboration with supervisor.
  • Follow all company policy and procedures found in the employee handbook, and other documents including but not limited to:
  • Employee and client safety;
  • Empower mission, vision and goals;
  • Characteristics of the population served;
  • Sensitivity to cultural diversity;
  • Empower policies and procedures manual, personnel policies in the Employee Handbook, standards of conduct, and those specific to individual job duties;
  • Confidentiality policy;
  • Reporting abuse and neglect policy and procedures in conjunction with other team members after collecting needed background information;
  • Client rights and grievance policy and procedures;
  • Follow state, federal, and local law, and all other policies and procedures that do not fall under the above responsibilities.

Physical Requirements: The physical requirements of the position are identified as moderate physical activity which may require the lifting of up to twenty (20) pounds occasionally and fifteen (15) pounds frequently.
