My Health Onsite operates onsite and near-site health and wellness centers. We deliver advanced personalized work-site healthcare solutions to employers that enhance patient engagement while proactively improving healthcare outcomes. Our medical team takes time to build strong relationships. No one is rushed in and out, and no one is a “number.” Patients may access a range of medical services including x-ray, an onsite pharmacy, wellness services, treatment for acute illnesses and chronic conditions. Our programs go beyond caring for the sick and injured – we make prevention our number one goal.

Summary:

Seeking LPN to work in collaboration with provider to provide patients with care, education and support to work in our Dade City health center. Serve as Wellness Advocate to promote healthy living, disease management and prevention through motivation, training and patient involved programs. Must be patient focused and demonstrate exceptional customer service and interpersonal skills.

Schedule: Monday, Thursday and Friday 6am - 3pm, Tuesday and Wednesday 7am - 4pm

Essential Responsibilities:

• Provide leadership and guidance to ancillary staff
• Welcome patients and escort to exam room
• Obtain and record vital signs (BP, heart rate, pulse oximetry, height, weight, waist circumference)
• Educate patients regarding their illness and treatment, including self-care, prevention and health maintenance
• Perform all nursing care duties, including procedures, injections, EKG's and phlebotomy
• Document all assessments, actions, care and treatment responses, and communications in patient record in EMR
• Maintain patient confidence and ensure confidentiality of patient care information
• Assist the physician and medical staff with procedures and treatments
• Promote wellness programs

Minimum Qualifications:

• BLS for Healthcare Providers
• Current and unrestricted State of Florida LPN license (or compact license that includes Florida)
• Understand CPT and ICD10 Codes
• Ability to perform standard phlebotomy procedures and techniques
• Proficient knowledge of computer systems including electronic medical record (EMR)
• Ability to effectively communicate with staff and patients using excellent written and verbal skills
• Friendly personality and ability to work well as a team member
