LPN Case Manager for 3 Young Adults

Seeking mission driven, creative and motivated individual to provide case management, advocacy and health and wellness support and education for 3 young adults in a new and innovative group home located in an apartment in Brighton. This program offers a therapeutic setting for young adults to develop the skills they need to successfully live in the community including skills for independent living, self-management, healthcare management, interdependence and self-determination. This program is a transition program that provides short-term support (3-5 years) for skill development and a scaffolded transition to their next steps.

The LPN provides medical case management, direct nursing care as needed, advocacy and health and wellness education for the medical/preventative health services for 3 young adult participants with complex health needs in a transition focused community-based residence.

QUALIFICATIONS

· Experience working with young adults

· Experience working with individuals with psychiatric conditions and/or co-occurring disorders

· Excellent interpersonal skills; ability to build positive and collaborative relationships with young adults, families, staff and external stakeholders.

· Required: LPN license

· Ability to read, analyze, and interpret client records, medical reports, professional journals, technical procedures, or governmental regulations.

· Ability to write reports, business correspondence, and procedure manuals.

· Ability to effectively present information and respond to questions from groups of managers, staff members, clients, other service providers, family members and guardians, and the general public.

· Ability to define problems, collect data, establish facts, and draw valid conclusions.

· Ability to interpret information from a variety of sources and make appropriate decisions in situations where limited standardization exists.

CERTIFICATES, LICENSES, REGISTRATIONS

Current Licensed Practical Nurse (LPN) License

Current CPR/First Aid certification

Current, valid, US drivers' license for at least one year.

All candidates must be able to successfully pass a CORI, reference, multi-state background and driving record check

Pay: $31.00 - $34.00 per hour
