Position Summary

The Intake Clinician is responsible for completing initial psychosocial assessments and diagnosis for children and adults with serious emotional disturbances, serious mental illness, and substance use disorders (SUD), in order to identify and refer to appropriate Swope Health Services behavioral health (BH) programs, SUD programs and medical clinics.

Primary Functions

  • Facilitate an assessment interview that produces sufficient information to identify a BH and/or SUD diagnosis, if such is present.
  • Ensure timely completion of initial psycho-social assessments for children, youth and adults requesting BH and/or SUD services that integrate physical and mental health concerns and identify strengths and functional challenges.
  • Assess for risk of harm to self or others and take appropriate action to ensure safety.
  • Make immediate referral, per protocol, for Swope BH and SUD services appropriate to support identified challenges.
  • Facilitate referral to Swope medical services to address any medical challenges identified.
  • Facilitate referral to other community resources necessary to address identified challenges.
  • Collaborate with other providers and/or programs to best meet client needs.
  • Complete required documentation according to established standards and time frames.
  • Attend and participate in department meetings as appropriate.
  • Participate in Performance Improvement activities related SHS Quality Performance Initiatives.
  • Must complete annual agency trainings as required by Swope Health Services.
  • Participate in continuing education to maintain licensure.
  • Maintain productivity standards as established by the organization.
  • Other duties as assigned.

Education/Qualification

  • Master’s Degree in social work (LCSW, LMSW) Preferred.
  • LSW eligible may be considered, dependent upon experience
  • Minimum of three years of professional experience working with seriously and persistently mentally ill (SPMI) adults and with seriously emotionally disturbed (SED) children, preferably one year in a community mental health setting.
  • Proficient knowledge and utilization of the DSM and the ability to assess mental health disorders and make person centered treatment and service recommendations.
  • Proficient knowledge of health and human services and community resources.
  • Motivational Interviewing knowledge and proficiency.
  • Trauma Informed Care awareness and consistent demonstration.
  • Excellent writing skills.
  • Strong consumer advocacy skills, with an empowerment philosophy.
  • Ability to navigate electronic medical records proficiently.
