  • The Housing Case Worker (HCW)/Relocation Case Manager (RCM) will use person-centered principles and practices to assist clients with obtaining and maintaining permanent housing.
  • This person-centered approach assures people with disabilities have the same rights and responsibilities as other people, including having control over their lives and making their own choices, and contributing to the community in a way that makes sense for the person.
  • The HCW/RCM provides assistance to clients by determining prospective housing options based on the client's preferences and budget, prospective housing amenities and location, and other financial resources that could assist with solidifying housing.
  • Meet in person with each client, complete intake and identify barriers that prevent them from finding, ransitioning and maintaining their own hosing.
  • Address barriers and work on building client housing goals.
  • Effectively communicates with clients and their care team to locate, plan and execute housing search and placement.
  • Meet with clients to discuss, evaluate and coordinate their applications for housing and other community needs thereby promoting independent living.
  • Schedule tours of housing options and accompany the client to housing-related appointments and advocates for them if need be.
  • You must be comfortable having to occasionally give rides to your clients for touring or as needed.
  • Assist in procuring necessary funds and resources.
  • Coordinate with movers and clients about moving to their new home and ensuring a smooth transition.
  • Assist clients understand the terms of their lease agreement as well as monitor housing and other acquired services in order to maintain stable stability.
  • Support various housing services.
  • Document all activities related to the client.
  • Keep client files organized and updated.
  • Other duties as assigned.
  • Have at least a 4-year Bachelor’s degree in Social Work, Sociology, Psychology or Human Services.
  • At least 1-year experience working in healthcare, human or social services.
  • A good knowledge of housing and other community resources around the Twin Cities, a plus.
  • Pass a Department of Human Services (DHS) administered background check.
  • Must be eligible to work in the United States.
  • Good communication skills including a good command of written and spoken English.
  • Ability to speak and/or write other international languages commonly spoken in and around the Twin Cities, a plus.
  • Have a valid driver’s license with current car insurance (proof required).
  • Ability to drive to multiple offices and residential locations.
  • Have compassion to work with people with mental, physical, developmental, and other disabilities.
  • Ability to embrace diversity.
  • Ability to effectively manage time and caseload to ensure punctuality to work, appointments, as well as meet deadlines.
  • Proficient in Microsoft office especially word and excel.
  • Ability to engage in extensive periods of computer work.
  • Ability to sit at a desk for periods of several hours.
  • Comply with HIPPA and Data Privacy Practices.
  • Positively represent agency, program and participant needs.
  • Ability to maintain a neat and professional environment.
