JSSA’s Hospice program is seeking a full time Patient Flow Manager to join our hospice leadership team. This role will report to our Hospice Director and oversee our Intake, Admission and Discharge teams and processes.

Responsibilities will include but not be limited to:

• Improve hospice throughput of admissions and allow for safe discharges from hospice into community care settings and reduce delays to appropriate levels of care
• Oversee staff members who work in the Intake department, Admission nurses, and nurses involved in our post-hospice Discharge Program
• Work closely with Community Liaisons and other team members within JSSA
• Recruit, train, and onboard new staff

Qualifications:

• Bachelor’s degree in Nursing (BSN) required
• Active RN license to practice in MD
• Minimum 2 Years of Hospice experience in a clinical role
• Experience managing and directing nursing staff
• Highly effective verbal communication skills including courtesy, resourcefulness and efficiency in answering questions, giving directions, locating staff and explaining hospice/departmental policies and procedures are necessary
• Highly effective process improvement skills including knowledge of data analytics and the ability to leverage technology
• Strong organizational skills
• Creates an environment that monitors and adapts to regulatory and practice changes
• Strong technical skills in telemedicine, Electronic Medical records and Microsoft Office 365
• Comfortable pulling reports and synthesizing operational data
