Currently looking for an experienced Hospice Case Manager to fill a permanent opening in the desirable Coastal Southern California area of Los Angeles. Interested candidates should have recent hospice nursing experience. Work a Monday-Friday Dayshift with very limited call, one time every 2 months.

Currently looking for an experienced Hospice Case Manager to fill a permanent opening in the desirable Montclair, CA . Interested candidates should have recent hospice nursing experience. Work a Monday-Friday Dayshift 8:30AM-5:00PM.

Responsibilities of the LVN Case Manager

• Responsible for the coordination of care for patient and family.
• Provide nursing evaluations and assessments.
• Develop a detailed Plan of Care for assigned patients.

Requirements of the Hospice Nurse Case Manager

• Recent Hospice nursing experience
• Licensed Vocational Nurse
• California LVN nursing license
