Optimal Home Health & Hospice Care is a trusted and growing agency servicing the Colorado Springs area. We are looking for an outstanding Community Liaison to join our team. Duties: What you will do:
- Build relationships with doctors, hospitals, nursing facilities, etc.
- Promote Optimal Home Health & Hospice Care as the premier home health & hospice and the best choice for both home care and end-of-life care
- Develop strategic sales plans resulting in increased hospice referrals and census growth
- Educate referral partners, patients and families about hospice and end-of-life care
- Assist patients and families with completing consent and other documentation
Requirements: What you need to have:
- Driver’s license, insurance and vehicle
- Ability to build relationships that result in company growth
