Primary Service Locations: All of San Diego and south Riverside county.
Work Locations: Escondido, Mission Valley, Carlsbad, Temecula

Status:
2 10 hour shifts
Weekends: Every weekend
Shifts: 10 Hour shifts - Day


The Elizabeth Hospice is San Diego’s oldest non-profit hospice of 40 years. We are growing and looking for talented individuals to join our high performing team and support our mission. We serve all of San Diego county and south Riverside county with multiple hospice office locations.

Responsibilities:
Hospice Field RN will see patients for symptom visits or change in condition, and complete new patient admissions when needed. Provides professional nursing care under assigned visits by Triage Nurse (phone) or LVN Scheduler for patients across the life span, (pediatrics, and adults) in patients’ homes or facilities according to established standards and practice.
  • Performs visits as needed and directed by the triage team.
  • Responds to requests for patient visits in a timely manner.
  • Independently assesses patient/family immediate care/symptom management needs.
  • Provides care and treatment as ordered.
  • Provides health care instruction to patients, caregivers and family members.
  • Other Hospice nursing visits as assigned.
  • Provides appropriate follow-up to case managers and team members in regards to patient condition changes and needs.
  • Responsible for the direction of care provided by LVN and HA in accordance with patient’s plan of care.
  • Responsible for the supervision of services provided by the LVN and HA per TEH policy and regulatory requirements.
  • Maintains timely patient documentation using "point of care" electronic charting.
  • Keeps current with all organization and department policies and procedures.
  • Collaborates with physicians and supervisors for changes in orders and changes to the patient’s level of care, as needed.

Qualifications:

  • Must have Associates Degree, Bachelor Degree preferred, and a minimum of 12 months experience with 1-2 years preferred in acute care; or equivalent combination of education and experience.
  • At least 12 months experience with 1-2 years preferred in acute care.
  • Hospice experience preferred.
  • Current CA RN license.
  • Bi-lingual (Spanish) preferred.
  • Excellent customer service skills.
  • Computer skills including Word, Outlook, and Excel.
  • Current California driver’s license with proof of insurance required.
  • Must be able to drive in various conditions, including but not limited to winding/steep roads, night driving, and rural areas, and be able to enter homes with pets.
