Current Opening for Monday - Friday 8 a.m. - 4 p.m.
The Granite VNA Hospice Community Aide performs personal care and designated health-related services for Hospice patients in their homes and/or in a facility. The Hospice Community Aide interacts with client’s family members, facility staff and other healthcare team members.
Essential Job Functions
Follows care plan in providing basic care and services to the resident including assistance with Activities of Daily Living (ADL): walking, bathing, shaving, mouth care, grooming, eating transfers, and management of incontinence.
Communicates with nurses involved in the resident’s care plan; observes, records and reports changes in resident’s condition, status, or other pertinent information to the nursing staff.
Does laundry, housekeeping and maintenance to assure a safe, orderly, clean and comfortable environment.
Provides supervision to patient/residents with cognitive deficit.
Ensures residents have the opportunity to engage in cultural, spiritual, physical, political, social and intelligent activities as the residents’ desire.
Maintains calm, rational manner when dealing with others, even in situations involving conflict or crisis.
Documents care provided to the patient/resident accurately in accordance to the agency’s guidelines.
Works independently on individual assignments yet support the needs of the total work unit.
Complies with the Agency’s documentation, time entry scheduling requirements
Communicates clearly and courteously with others including client, family members, and other agency staff.

- Eligibility - employed 20 hours or more a week

Requirements: High school diploma/GED, accompanied by current New Hampshire LNA license
Reliable automobile and automobile Insurance
Valid Driver’s license
Proof of completed COVID-19 vaccine
