Position Summary:

The Client Services Representative (CSR) performs outreach to homeless clients to inform them about health and social service programs. Position is also responsible for acting as an advocate and liaison; assists with obtaining healthcare coverage, healthcare services and access to other resources. Compensation is dependent on skills and experience.

Your contribution to the team includes:

  • Identify and screen clients in shelter who may be eligible for assistance with their health care and other social services. Inform and educate clients about their eligibility for programs, facilitate the application process and assist in accessing applicable services.
  • Know and be able to clearly communicate information on state and federal assistance programs including Medicaid, Medicare, Social Security Disability and Veterans Administration programs to clients who are homeless or at risk of homelessness. Assist client in recertification for Medicaid (Apple Health).
  • Monitor and track health insurance application process and provide follow up. Communicate with the community health shelter nurse at least daily to coordinate service delivery for optimal outcomes.
  • Provide referrals to homeless clients for health care, housing, transportation, basic benefits, income, and employment support as identified by the client.
  • Collaborate effectively with other agencies and service systems to assure client continuity of care. Establish trusting relationships with partner agencies. Maintain patient confidentiality and obtain appropriate releases of information (ROIs) as needed. Inform shelter nursing team of newly identified resources in a timely manner.
  • Assist clients with community navigation by meeting them at agencies to which they’re being referred and accompanying clients to clinics.
  • Assist in the development and implementation of outreach activities and materials to enhance and improve customer service and client relationships through internal and external agencies.
  • Identify appropriate community resources and develop relationships through marketing, networking and outreach activities.
  • Achieve targets for productivity (encounters, number of clients served, number of referrals, etc.). complete documentation in a timely manner in accordance with HealthPoint standards. During client interactions, will utilize motivational interviewing principles, utilize trauma informed care and respect cultural differences.
  • Maintain good attendance, is punctual and works full scheduled shift is a condition of employment.
  • Demonstrate respectful, professional, and appropriate behavior that supports a team-oriented work environment.
  • Demonstrate a commitment to the mission, core values and goals of HealthPoint and its healthcare delivery including the ability to integrate values of justice, respect, compassion, excellence and stewardship into appropriate programs and services.
  • Attend staff meetings, in-service meetings and participate in agency committees and task force activities as required.
  • Other duties as assigned by supervisor.

Must have’s you’ll need to be successful:

  • Associate’s degree (AA) or equivalent from a two-year college or technical school and one year experience in a medical setting; or equivalent combination of education and experience.
  • Possesses basic operating knowledge of computers. Beginning level of Word and Outlook required to start. Must be able to learn to work efficiently in NextGen electronic medical record.

Proof of immunizations and/or immunity to: COVID-19, MMR (measles, mumps, and rubella), Varicella, Annual Influenza, and TB are required for all employees. Additionally, if you work in a HealthPoint clinic, Tdap (within last 10 years) is required. Hepatitis B. is required for clinical employees with potential exposure to blood/blood products. All immunizations are a condition of employment. Upon hire, employees must provide proof of their immunizations and/or immunity prior to starting or no later than their fifth (5) business day of employment.
