Home health care nurses have a unique opportunity to interact directly with the patient and the patient's family. Health is something that happens at home, not only at the doctor's office. They have the chance to provide comprehensive, one-on-one care in their own environment.
Generally, our nurses have the freedom to set their own schedule without following the strict shift structure associated with traditional hospital or skilled nursing facility settings. And although home healthcare nurses are a part of a larger care team, you do have autonomy and can be independent.
The ideal candidate will:
- Have a natural tendency to care deeply about their patients
- Must be able to pay attention to detail, be able to solve problems, and have the willingness to learn.
- Have a strong communication skills allowing effective communication between other nurses, physicians, other disciplines, as well as patients, and their families
- Have a good sense of humor also helps spread positivity to other nurses, patients, and their families

You will be responsible to:
- Complete an initial assessment of patient and family to determine home care needs.
- Provide a complete physical assessment and history of current and previous illness(es).
- Act as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
- Participate in management and interdisciplinary team meetings for the purpose of information exchange on patient care planning and to improve our programs.
- Communicate with the physician (or other authorized licensed independent practitioner) regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s (or other authorized licensed independent practitioner) orders as required.
- Develop a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
- Act as Vision Home Health Care representative at institutional and community programs and functions.
- Participate in special projects and performs other duties, as required.

Salary: $80,000.00 to $140,000.00/year
Experience:
- Nursing: 2 years (Preferred)
- Home Health and/or Hospice: 1 year (required)
