Home Health Marketing/Community Outreach Position available!

Community Home Health Care Services is looking for an outgoing, charismatic, professional individual to help grow and support its relationships with other Healthcare Professionals.

Responsibilities include:

• Identification of new potential patient referral sources.
• Development and maintenance of existing and prospective professional relationships/accounts.
• Care Coordination to ensure fluid transition of patient to/from other providers.
• Visit and keep current referral sources. We do receive referrals from providers in Sarasota for home bound patients in Manatee County, including Myakka, and the nearby rural areas.

Individuals applying should be professional, outgoing, and comfortable with meeting new people every day. Must have reliable transportation and willing to drive most of the workday. Marketers will be reimbursed for their mileage.
