Job description
The Forensic Case Manager provides ongoing supports to Forensic clients to assist in the development of the client’s treatment plan, assessments and the provision of support in assisting FSP clients in locating, applying and maintaining appropriate housing as well as assisting individuals in applying for benefits such as SNAP, Social Security and other benefits or programs the Forensic client may be eligible for.
The Forensic Case Manager under the direct supervision of the FSP Coordinator, The Forensic Case Manager is responsible for working as a member of the FSP team to provide case management services to adults with mental illnesses and who are involved in the criminal justice system.
Minimum requirements of the Forensic Case Manager include a Bachelor’s degree in human services and experience working with adults with serious and persistent mental illness. Valid driver’s license and ability to drive company vehicles (including vans) to safely transport clients.
Job Type: Full-time
Pay: $34,000.00 - $35,000.00 per year
