This Company is contracted with the IL Department on Aging to provide non-medical, in-home services to low-income seniors as part of the State's Community Care Program.

We are seeking to fill the position of Field Supervisor, whose primary responsibility will be to conduct visits to senior's homes to ensure the delivery of quality in-home services through our Homecare Aides.

Key Responsibilities:

• Ensure that the agency is in compliance with all applicable Federal, State and Local Laws. Be familiar with the rules of the Agency and maintain them within the agency.
• Make written and oral reports/recommendation to the Executive Director, as necessary/required, concerning the operation and policies of the private duty agency.
• Assist the client and family in planning the clients care while in the home.
• Perform administrative duties such as completing forms, reports, evaluations, studies, charting, etc., as necessary.
• Planning, preparing and documenting contact and quarterly conferences with each assigned homecare aide.
• Assures the provision of high quality services needed and/or desired by client.
• Establishes priorities consistent with agency goals.
• Evaluating each assigned homecare aide annually;
• Coordinating the homecare aide's activities with other components of the Plan of Care as required;
• Making and documenting semi-annual/quarterly in-home supervisory visits to a client's home for each assigned homecare aide;
• Making home visits, as necessary, to provide hands-on training and assistance
• Continues to expand his or her knowledge through attending educational seminars and/or special programs pertaining to private duty.
• Demonstrates an appreciation for the heritage, values and wisdom of the client and an understanding of the aging process.
• Develop work assignments and schedule duty hours as appropriate.

Qualifications:

• a high school diploma or general education diploma;
• combination of skills and experience that indicate that the applicant has the ability to perform the supervisory activities; and
• certification of completion of Department sponsored CCP training required by subsection (a)(3)(A).

Training Requirements:

• Within 90 calendar days from the date of employment with the provider agency in a homecare supervisor position, each supervisor shall complete Department sponsored CCP training on policy and procedures, billings, evaluations, homecare aide and client files; and
• Within each calendar year, each supervisor shall complete 24 hours of documented in-service training on aging related subjects, including documented participation in in-house staff training and/or local, State, regional or national conferences.

Job Type: Full-time

Pay: $32,598.00 - $45,000.00 per year
