Family Case Manager
EAC Network, a not-for-profit social service agency that empowers, assists, and cares for over 65,000 people in need through 100 programs across Long Island and NYC, seeks a Family Case Manager to work full time for its Suffolk Judicial Diversion Court Program.
The Family Case Manager position is an non-exempt position paying $30 per hour at 35 hours per week. The hours of operation are Monday-Friday 9:00AM-5:00PM and the office is located in Riverhead, NY. The role reports to Program Director.

Primary Purpose of Job:
The Family Case Manager is responsible for helping family members of Judicial Diversion Program (JDP) participants become educated on the addiction/recovery process and link family members to supportive services.
Principal Duties & Responsibilities:
- Provide a full range of information, support, encouragement, advocacy, and referral to appropriate community resources, and related services to assist the family members cope with JDP participant’s SUD.
- Assist in conducting assessment interviews, and evaluations for clients referred to the program.
- Prepare written reports in a timely manner regarding client’s status with the program treatment plan and progress.
- Develop appropriate treatment plans for clients accepted into the program and facilitate placement.
- Give families detailed information about the behavioral health service system; how it works and services available for family members.
- Educate families on privacy and confidentiality regulations, HIPAA, AB1424, AB1194, patient and family rights, and the release of information (ROI) process
- Promote ongoing self-care techniques and wellness and recovery activities for families and their loved ones.
- Virtually facilitate and monthly family caregiver support groups.
Knowledge, Skills, & Abilities Required:
- Master’s degree in a helping profession (e.g., counseling, criminal justice, psychology, social work, or a specialized field like addiction), participation in at least 12 hours of qualified training in substance abuse, a minimum of two years of experience working with the substance abusing population required.
- Should have comprehensive knowledge of the family dynamics of addiction.
- Must have good communication skills, both oral and written. Ability to work cooperatively with criminal justice systems.
- Ability to handle difficult situations in a professional manner.
- Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, email and/or database software necessary to perform work assignments
