Responsibilities
Discharge Planner - Full Time

Discharge Planner / Care Coordinator:
The Discharge Planner/Care Coordinator is responsible for planning and coordinating activities related to discharge planning. Developing and implementing a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care.
Performance Standards / Duties:
- Communicate with referral source and school, if applicable, within the first 24 hours of admission.
- Obtain all releases of information necessary to coordinate care.
- Review all new admissions; meet with patients admitted from the previous day and complete the discharge planning assessment
- Review cases with the Clinical Therapist involved, manage the needs of the patient throughout treatment experience, and determine placement outcomes
- Attend treatment plan meetings to discuss and determine all discharge planning needs as indicated by patient, family, and treatment team
- Communicate all discharge planning and patient status/progress with the patient, family, outpatient community team, and Utilization Management
- Regularly communicate with outpatient providers
- Utilize and assist in developing a file of resources that identifies social service resources available in the community for patients
- Educate patients on the need for aftercare placement and assist patient with phone calls, appointments, and aftercare needs
- Properly document progress toward discharge and discharge plans in medical records and file as needed
- Maintain the discharge calendar to assure discharge plans arranged prior to patient leaving the facility
- Work in a group setting as required for programming.
- Participate in care coordination meetings with community providers and payor sources, such as Colorado Access.
Highlands Behavioral Health System is an 86 bed, acute care psychiatric hospital located in Littleton, CO. Highlands features individual units for adolescents, adults, and seniors, and offers inpatient acute care, partial hospitalization, and intensive outpatient programs.
Qualifications
- Master’s Degree from an accredited college or university, in Social Work or a clinical related mental health field preferred.
- Minimum of one year experience in a psychiatric or mental health setting.
- Direct experience in patient/family assessment or discharge planning preferred.
- Crisis Prevention Certification Trainings (Handle with Care) within 30 days of employment and prior to assisting with a restraining procedure. Certification training is offered during new hire orientation.
