We invite you to join our highly dynamic team as a Diabetes Educator II.
As a Diabetes Educator II, you will be responsible for:
- Serves patients with diabetes, and those at risk for diabetes and other chronic disease.
- Performs outreach, disseminates information, and provides self-management support.
- Participates in a variety of community events and health fairs.
- Supports an integrated, multidisciplinary team-based approach to patient care, families, and communities.
- Provides education on a variety of health topics with a focus on prevention of chronic disease and self-management skills.
- Provides assistance with access to health services as appropriate.
- Responsible for maintaining a list of community contacts and referrals.
- Assists with development, review, and revision of policies, procedures, protocols, curriculum and materials.
- Supports diabetes coalition building to provide more access and resources for people with diabetes.
- Supports advocacy for people with diabetes.
- Operates within professional lines of responsibility and communication.
This is a Full-Time position with an 8-hour shift with variable hours and days.
Preferred Education/Experience/Specialized Skills/Certification:
- Experience teaching, health coaching or promoting community health.
Required Education/Experience/Specialized Skills:
- Ability to interact effectively with diverse groups.
- Demonstrated experience and ability to work independently and within a team-based structure.
- Ability to set priorities and goals to ensure program objectives are met with minimal direction.
- Ability to handle problems, propose solutions, and implement them.
- Experience in teaching or instructing individuals and groups.
- Experience using Microsoft office.
Required Certification/Registration:
- Current BLS for Health Care Provider from American Heart Association.
- One of the following:
- Certified Community Health Worker
- Medical Assistant
- Dietetic Technician
- Registered Pharmacy Technician
- Licensed Vocational Nurse
- Diabetes Prevention Program Lifestyle coach certification.
