Emergency Department Case Manager

Position Overview:
The ED Case Manager is responsible for coordinating ED patients’ access to the appropriate level of care to a population of different race, creeds, religions and ages within the Singing River Hospital System. He/She focuses on clinical quality and resource management resulting in defined clinical, satisfaction and financial outcomes. Expectation is for all performed duties to be in accordance with Singing River Health System procedures and policies, accreditation organization, and governing guidance and publications for health care employees.

Education:
Must be a graduate of an accredited School of Nursing, Bachelor’s degree in Nursing preferred, or Bachelor’s Degree in Social Work required, Master’s Degree in Social Work preferred.
License:
If a Nurse, licensed to practice as a Registered Nurse in the State of Mississippi. If a Social Worker, licensed to practice as a Social Worker in the State of Mississippi.

Experience:
Acute care experience within the previous 5 years that has relevance to the practice of Case Management Acute care experience within the previous 5 years that has relevance to the practice of Case Management. ED clinical experience preferred.

Physical Demands:
Stands, turns, bends, stoops, lifts, climbs, and uses many physical motions in performing day-to-day activities. Must be able to stand for extended periods and do a great deal of walking without experiencing undue fatigue. Potential exposure to communicable disease or radiation exists.
Work involves being able to perceive the nature of sounds at normal speaking levels, with or without correction. Must demonstrate the ability to communicate, speak, and enunciate to express/exchange ideas and detailed information in person and on the telephone. Work requires a minimum standard of visual acuity, with or without correction, with the ability to adjust the eye to bring an object into sharp focus, i.e. shift gaze from viewing a computer monitor to forms that are closer to compare data at close vision.
Mental Demands:
Must demonstrate keen mental faculties/assessment and decision making abilities. Must demonstrate superior communication/speaking/enunciation skills to receive and give information in person and by telephone. Must demonstrate strong written and verbal communication skills. Must possess emotional stability conducive to dealing with high stress levels. Must demonstrate ability to work under pressure and meet deadlines.
Attention to detail and the ability to multi-task in complex situations is required.
Special Demands:
Must possess superior customer service skills and professional etiquette. Must possess proficient knowledge and ability to use a computer (must be keyboard proficient) and other office technology (i.e., telephone, fax, etc.). Must have working knowledge of MS Outlook, Word, Excel and PowerPoint.
Job requires traveling throughout the SRHS service area – with the employee providing his/her own transportation. Must have a valid driver license.
Work requires the ability to function independently, adapt to workload demands, set priorities, and understand and set goals. Must possess highly developed organizational, planning and management writing skills. Must understand the fundamentals of automated data processing and gain a detailed understanding of complex computerized and non-computerized information within designated period.
