Local non-profit health care center is looking for a Counselor (LPC, LMSW, or LCSW) for their Port Arthur office. Working on either a full-time or part-time basis, the Counselor will initiate and complete comprehensive assessments, provide individual, group, and family therapies to clients and their families. This full-time position offers the potential for growth, a friendly staff, and an opportunity to enhance the health of our dedicated patients.

Responsibilities:
- Conduct comprehensive assessments that identify behavioral, clinical, social, and environmental concerns and needs of patients;
- Based on this assessment, and in conjunction with the patient, the patient's physician and other members of the population health team, work with the team to create a care plan that will address the identified needs, remove the barriers, and improve the health of the patient;
- Create a comprehensive inventory of local community and government resources for patients and their families and facilitate the patient's access to these resources;
- Assess the patient's knowledge of their clinical condition and provide education and self-management support based on the patient's unique learning style.
Qualifications:
- Licensed LPC, LMSW, or LCSW in Texas
