SUMMARYFacilitates patient access to clinically appropriate entity and level of care in collaboration with other members of the health care team using clinical criteria, utilization management and managed care processes. Responsible for coordination of patient transfers and placement. Serves as leadership resource as needed.
ENTRY REQUIREMENTS
1. Graduate of an accredited school of nursing. Currently licensed as a Registered Nurse in the State of Michigan. Bachelor's Degree required; Master's preferred.
2. Minimum of 5 years' nursing experience, with at least 1 year experience preferred from ICU, ED, or Telemetry units.
3. Demonstrates administrative and supervisory ability.
4. Demonstrates ability to lead/direct others and to work effectively with persons of varied background and educational experience.
5. Demonstrates ability to establish excellent working relationships with physicians, hospital leadership, and all levels of staff.
6. Demonstrates team spirit, dependability, initiative, openness to learning new things, and good judgement to interact effectively with team members.
7. Demonstrates effective written and oral communication skills
8. Demonstrates ability to work in a demanding and stressful environment, where prioritizing is essential while maintaining a professional demeanor in difficult situations.
9. Additional education in management principles is preferred.
ORGANIZATION
- Reports to the Director of MHC System Transfer Center.
- Works collaboratively with all physicians, nursing leadership and healthcare team members across the system.
- Functions within approved nursing, medical and Munson Healthcare standards and policies.
SPECIFIC DUTIES
1. Serves as the primary, centralized owner of patient placement and transfer for the Munson Healthcare system.
2. Requires system thinking that spans the patient's continuum of care.
3. Exercises clinical judgment to collect clinical data to determine appropriate facility placement and level of care.
4. Communicates and collaborates with the interdisciplinary team to develop and implement plan for patient placement and transfer.
5. Facilitates patient placement process, including problem solving, in order to expedite and achieve clinically appropriate and operationally effective outcomes.
6. Provides ongoing updates of pending and confirmed transfers.
7. Identifies options and facilitates transfers to higher level of care when needed.
8. Partners with charge nurses to identify and problem-solve patient placement barriers to expedite bed assignment process.
9. Manages multiple competing tasks with energy and efficiency.
10. Demonstrates accountability for maintaining current knowledge of rules, regulations, policies, and procedures. Serves as a resource to staff.
11. Maintains good rapport and cooperative relationships.
12. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution.
13. Demonstrates area designated competencies.
