JOB SUMMARY
The Social Worker (SW) provides assessment, planning, problem resolution and casework services to patients and their families according to age specific needs. The SW collaborates with the referring physician when needed and completes all patient care documentation concisely and timely. The SW will assist with the ongoing discharge planning needs of the patient. The SW is available for patient/family referrals upon the request of the physician and/or patient. The SW is to provide in-hospital consultations within 24 hours of notification by the hospital. The SW is available by phone for consultation as requested by NCSC hospital staff, physicians or patient/family.
DUTIES AND RESPONSIBILITIES
- Consults with Clinical Manager/CNO re: policies/procedures and opportunities to improve service.
- Maintains statistical data including activity reports, quality improvement reports, and other administrative reports as indicated.
- Services as a liaison with community agencies, organizations, or groups to promote services and opportunities for the health care consumer as delegated.
- Provides direct patient care services upon referrals, requests for physicians and patients/families. Effectively interviews patients and families and assesses psychosocial status, family relationships and post hospital care needs when indicated.
- Identifies patient and family problem areas and develops appropriate treatment plan.
- Participates in team conferences and family meetings by providing relevant information and collaborating with the treatment team regarding psychological adjustment and continuity of care needs or concerns as requested.
- Provides support and individual counseling services to patient and family in relation to health, economic, and social needs.
- Provides assistance in crisis situations.
- Provides ongoing emotional support to enable patient and family to adjust to limitations or disability.
- Collaborates with patient, family and interdisciplinary team in determining discharge planning needs for individual patient.
- Provides community referral sources for economic, legal, transportation, housing, employment assistance, and other identified needs as indicated. Keeps notebook of resources updated.
- Provides social histories as requested by physicians with resolution of conflicts/concerns.
- Refer issues to appropriate department for follow-up/resolution.

POSITION QUALIFICATIONS
- Masters or Bachelor degree of an accredited program of Social Work.
- Five years hospital experience is required. Experience with all age groups preferred.
- Licensed Medical Social Worker in the State of Texas
- Possess the ability to deal tactfully with staff, physicians, patients, families and visitors.
- Knowledge of TJC, CMS, TDH standards, community resources, and third party payor sources.
