The Community Liaison is responsible for implementing person-centered strategies, by supporting individuals with disabilities, to increase meaningful relationships in their community, inclusion in the community, and membership within their community. Engaging in community life includes any activities of volunteering, continuing education, business, recreational, faith based, participating in clubs or getting involved in groups by exploring new hobbies and areas of interest. This person must be knowledgeable about the individuals supported, their social networks and supports, and the opportunities to expand their personal relationships, for individuals with intensive challenges. This person will work with individuals and support persons to develop relationship maps and enhance these strategies. The Community Liaison will also focus on finding jobs for individuals in the community. They will contact businesses, create plans for individuals to pursue employment, that they are interested in, and work with employers to engage in job trials and potentially gain employment.

ESSENTIAL JOB FUNCTIONS

Responsible for the health, safety, and well-being of developmentally disabled clients in Day Program facilities:

1. Work closely with individuals, their families, and their natural supports and the community to develop opportunities in which the individual will be successful.

2. These opportunities will include assessments of their strengths and interests, identification of activities that will help the person grow socially aimed at achieving the goal of meaningful participation in their communities, expansion of natural personal supports and identification of employment opportunities that are strength based and rewarding.

3. Assist the individuals and others in their life in identifying where they would like to be in the future, and develop a plan to achieve that vision and those goals

4. Work in conjunction with the Day Support staff to increase individual matching of personal interests and preferences to identify resources and opportunities in the community to promote recreational, vocational development and physical and behavioral health for each participant

5. Create a learning environment; develop strategies to help the individuals cope effectively with social, emotional, and physical situations in daily living and facilitate independent performance in these areas.

6. Implement Individual Plan of Care/Follow and follow as based on the Interdisciplinary Team’s identification of client needs.

7. Coordinating and monitoring effective supports

8. Monitor medical, physical and behavioral changes in individuals and report changes to the R.N. or Program director as it occurs, is observed or discovered. Write up incident report.

9. Provide training and mentoring to new and existing employees

10. Writing reports/updates on programming

11. Maintain individuals right to privacy, and adhere to CRI’s confidentiality Policy.

12. Must be able to utilize approved protective hold techniques in order to keep you, clients, and co-workers safe; works with youth who may potentially cause harm to you or others.

13. Physical Demands: must be able to lift and transfer 70-75 lbs. as work is active and performed inside and outside of the facility.

14. Participate in training opportunities (in-service trainings, staff meetings, conferences, etc.) as required; maintain all required training per agency policy and BHDDH regulatory requirement.

OTHER REQUIREMENTS:

1. Valid RI driver’s license and a good driving record required.

2. Must be able to become certified in CPR and First Aid.

3. Demonstrate organizational ability.

4. Demonstrate ability to communicate effectively with adults who are developmentally disabled

5. Demonstrate ability to supervise and guide other staff while acting on behalf of the Program Director

6. Demonstrate knowledge of current issues in the field of Developmental Disabilities.

OTHER QUALIFICATIONS

1. Display motivation and interest in improving quality of life for the population served.

2. Display interest in being part of a multi-disciplinary teaching approach.

Job Type: Full-time

Pay: $18.00 - $20.00 per hour
