POSITION DESCRIPTION

Title: Community Health Representative

Schedule: Monday – Friday 8am – 5pm

Location: Longview

Classification: Non - Exempt

Opening Date: November 9, 2022

Salary Range: $24.89 - $32.36 per hour

Closing Date: Open until filled, first review November 23,2022

Position Summary:

Provides ongoing health promotion and disease prevention. Improves the health knowledge and status of patients by promoting, supporting, and assisting the Tribe in delivering a total health care program. Supports providers by providing patient outreach, limited transportation, scheduling, reminders, and health promotion activities. Honors and represents the mission, vision, values of CIT and HHS department.

Essential Job Duties and Responsibilities:

  • Adheres to the direction of the medical care and client coordinated plan, which lists goals and actions required to meet the needs of the client.
  • Documents all patient encounters in the Electronic Health Record (EHR).
  • Communicates with medical team.
  • Performs home visits to assess patient’s health and healthcare needs to promote healthy living.
  • Reports concerns or abnormal findings to medical team to meet the patient’s needs and to coordinate appropriate services and/or resources.
  • Advocates for the community health care needs and services.
  • Assists in providing quality health promotion and disease prevention activities.
  • Assists community members/patients in accessing appropriate medical care in a timely manner.
  • Provides resources to assist clients with their healthcare and social needs.
  • Acts as a liaison for the community to facilitate information about clinical services to meet community needs.
  • Collaborates with other departments to maintain communication and good working relationship within the organization to ensure quality health care service for patients.
  • Assists patients with travel education and transportation access.
  • Contributes to a team effort and accomplishes related results as required.
  • Performs other duties as assigned.

Knowledge, Skills, and Abilities:

  • Knowledge of traditional form of government and tribal customs and traditions.
  • Knowledge of health-related issues, medical terminology, and health and wellness education.
  • Knowledge of policies and procedures related to HIPPA, Medicare, Medicaid and insurance programs.
  • Skill in operating various word-processing, spreadsheet and database software programs in a Windows environment including electronic health care systems.
  • Skill in socializing and working with patients.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to handle multiple tasks and meet deadlines.
  • Ability to exercise independent judgment.
  • Ability to establish and maintain professional relationships with individuals of varying social and cultural backgrounds and with co-workers at all levels.
  • Ability to maintain confidentiality.

Minimum Qualifications:

  • High School Diploma/GED.
  • Two years' experience in community health, social work, social services, community advocacy, and/or community outreach in a healthcare setting.
  • A combination of relevant education and related work experience may be considered.
  • Must successfully compete CHR training with HIS within one year of employment.
  • Must possess or obtain CPR and First Aid certification within ninety (90) days of employment.
  • Valid driver’s license is required when driving vehicles for work-related purposes.

Requirements:

  • This position requires qualified applicants to be fully vaccinated including COVID-19 vaccines on their first day of employment.
  • Must be able to successfully pass a background screening / investigation according to the established requirements below

Tribal preference in accordance with Cowlitz Indian Tribe Code and CIT Employee Policies and Procedures Handbook shall apply to all positions. All positions are required to adhere to CIT’s established safety policies regarding vaccine requirements and personal protective equipment (PPE).

Physical Demands:

While performing the duties of this job, the employee regularly is required to sit; stand; walk; use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. The position requires occasional lifting of up to 50 lbs.

Work Environment:

Work is generally performed in an office, home, elderly, or community setting with a moderate noise level. Employee may be exposed to unpleasant odors, bodily fluids, and infectious disease with frequent exposure to heat, cold, fumes, and steam. Evening and/or weekend work may be required. Tight time constraints and multiple demands are common. Travel is required regularly.
