Job title: Community Health Worker
Reports to: Community Health Project Manager (NCOA Grant)
General Position Overview: The Community Health Worker will be responsible for the outreach, education and dissemination of COVID-19 and Flu information to disproportionately impacted populations. Outreach, education, and dissemination efforts shall aim to:
1. Educate and increase an individual’s knowledge about how to prevent the spread and mitigate the harm of COVID-19 and the Flu.
2. Build an individual’s skills to mitigate the risk of harm from COVID and the Flu.
3. Change an individual’s attitude and perception about COVID-19 and the Flu.
4. Change an individual’s commitment to employing harm reduction practices and behaviors.
Main Responsibilities:
- Manage patient wellness checks- inputting all necessary information in database.
- Gather new patient history and complete initial intake/assessments.
- Work with the Health Administrator on ensuring monthly goals are being met.
- Support and recruit patients on an ongoing basis.
- Follow up with patients on a regular basis to ensure success and completion of the CBSI program.
- Provide patient education and blood pressure resources for each cohort.
- Provide information assistance and referrals to community-based programs including ONEgeneration services.
- Conduct outreach in the local community area (San Fernando Valley) and recruit patients for the CBSI program.
- Manage data input for those enrolled in the CBSI program. (Patient progress)
- Must be able to type and process assessments on a computer/lab top. (Electronic health record)
- Some familiarity with HIPAA privacy practices rules/regulations. Additional training provided.
- Support the Health Administrator and Physician Assistant as needed.
- Establishing community partners and conducting ongoing outreach to enroll new patients into the program. Target population- low-income older adults.
Qualifications:
BA/BS degree in Social Work, Sociology, Gerontology or Psychology; Self Starter, Poised, Communicative and Articulate; Skillful written communications; BILINGUAL REQUIRED-SPANISH SPEAKING.
Skills: Microsoft Office – Microsoft Word, Excel, Power Point

Working Conditions:
This position is remote. All COVID protocols and practices are mandated and must be followed by all staff. Work may occasionally require weekend and evening work.

Pay: $22.00 - $25.00 per hour
