Job Purpose

This position assists Child and Family Services in establishing effective relationships and workflows with the community healthcare systems including but not limited to health plans, pediatric and family practice; hospitals, schools, and OB/GYN practices. The director will develop relationships with partners to establish opportunities to integrate best practices and collaborative care opportunities. This position will also support community alignment needs of the Family Connect program.

Duties performed by this position will be executed in a manner that will accomplish the mission and principles of the CFS programs, in compliance with the CFS grants and funding sources.

This position requires use of the employee’s education, training, and expertise to make decisions.

This position fully embraces the CFS mission and 7 key principles, as well as the Standards of Quality of NFSN.

Essential Functions

Establish relationships with healthcare systems and providers both internal and within the community to provide innovative collaborative care opportunities.

Develop Collaborative Care Agreements with partners to ensure communication and services are effective and have sustainable funding streams.

Develop workflows and systems to establish integrated care within school systems with school nurses to assist in providing telehealth psychiatric services.

Assist in setting up groups in healthcare entities/ organizations for children and parents to discuss mental health challenges and coping skills to handle challenges:

Collaborate with health plan partners to determine gaps in services and foster collaborative responses to challenges concerning mental health in schools.

Coordinate contracts with schools, healthcare entities, health plans that promote quality through alternative payment methodologies

Collaborate with medical providers on developing new and innovative practices that support the child in a holistic manner and within a family unit.

Create procedures and practices for jointly serving youth with the primary care physician through training, consultation, and joint visits

Serve as primary contact for community and school healthcare administration

Participate on healthcare plan forums to stay abreast of Medicaid rules and funding streams

Provide oversight for HMT Healthcare navigators

Provide oversight for the HB 19 services rendered in partnership with the Educations Services Center

Coordinate services with the school navigators to help serve in schools that have behavioral health

Support the multidisciplinary diagnostic team in creating integration of healthcare services for children with autism services into the school and primary care practice

Serve as the primary contact for healthcare contract entities: UTSW, Scottish Rite; Family Connects International

Develop and maintain relationships with community resources, school districts, stakeholders, and parents to promote Child and Family Services and MHMR.

1) Promote CFS activities by attending events, networking opportunities and individual outreach.

2) Present as needed to the Early Childhood Wellness Council and Youth CAC.

Attend subcommittee or work group meetings as assigned.

Q) Performs tasks necessary to support the goals and objectives of CFS programs.

Manage daily tasks and long-term goals so that CFS aligns with service area communities.

Maintains and submits all required daily documentation.

Ensures work performed benefits the best interest of CFS programs and the children and families it serves.

Meets standards and requirements of tasks, as assigned.

Completes assignments in a professional and timely manner.

Participates in meetings and committees, as requested.

Provides resources and support, when needed.

Ensures assignments and reports are accurate and deadlines are met.

Uses personal judgment for appropriate verbal and physical interactions with community relationships, staff children and families.

Maintains discretion and confidentiality at all times.

Meets performance measures/expectations as determined and outlined by supervisor.

R) Initiates professional growth and development and maintains competency through continuing education.

Successfully completes orientation and professional development within required timelines.

Successfully completes all required MHMRTC and including annual refresher courses, within specified timeframes.

Successfully completes, passes and maintains all required NSFN training and 2-year renewals.

Attends professional meetings, webinars, or seminars, when requested or as approved.

Participates on MHMRTC or community committees, as agreed upon by supervisor.

Performs other duties and special projects as assigned supervisor.

General knowledge of typical child development, disordered and atypical development, and medical diagnoses and conditions impacting children birth to six years old.

Knowledge of CFS programs’ eligibility requirements and how to refer families and providers

Knowledge of CFS grants and funding sources.

MHMRTC Policies and Procedures.

FERPA and HIPPA compliance.

Good presentation skills.

Minimum Qualifications

Education: Bachelor’s Degree in Nursing

Preferences: Marketing and Leadership experience; Business and Clinical experience

Agency Requirements

All staff are required to participate in agency Emergency Preparedness and Environmental Safety programs and may be assigned by their department as a key/essential staff level function during critical events or for the purpose of sustaining business continuity.

This position may require temporary or permanent re-assignment to any MHMR Tarrant facility as determined by program needs and/or the Division/Director.

Assigned work hours may change as the needs of the agency change.

The Functional Title of this position may change as the needs of the agency change.

All work will be completed within the scheduled work hours. All non-exempt (hourly) employees are expected to clock in and clock out for each work shift, no work should be done off the clock.

Prior approval from supervisor is required for all Paid Time Off (PTO) and Overtime.

MHMR reserves the right to change, add to or eliminate positions as it deems appropriate.

Employment is at will, as well as agency needs may change.

Agency dress code is to be followed at all times.

Regular and predictable work-site attendance, and punctuality is an essential function of this position.

Physical on-site presence, including regular attendance and punctuality, is an essential function of this position. Any changes or adjustments to your assigned work schedule or shift hours must be approved by your supervisor in advance.
