Job Title: Community Care Network Program (CCNP)- Family Development Coach

Job Status: Full Time / 35hrs per week

Salary Range: $45,000-$55,000
The purpose of Neighborhood SHOPP Community Care Network Program (CCNP) is to assess participants’
individual needs to increase access to benefits and entitlements and provide educational and recreational activities
which build on individual strengths. CCNP aims at increasing access to participants and promoting positive physical,
psychological and social well-being.
Services to be Rendered:
The Family Development Coach- CCNP is specifically responsible for the following tasks:
1. Visit SHOPP sites on a weekly-scheduled rotation basis to conduct outreach and registration of
clients and to monitor participation in program;
2. Conduct a) holistic assessment and development of a Family Development Plan, outlining the
participant’s strengths and self-defined goals, steps toward achieving the goals, available
supports, and resources needed; b) implementation of the Family Development Plan, i.e.,
coordinating community resources and working with and advocating on behalf of the participant to
meet identified goals; c) regular review of the Family Development Plan to assess whether goals
have been met or need to be changed.
3. Screen clients for benefits, entitlements, and services; assess need and eligibility and assist
participants in applying for needed benefits, entitlements, and services; provide follow up to
ensure services are obtained; provide supportive counseling as needed.
4. Provide information and referral to other appropriate agencies or SHOPP services as needed and
identify community resources for consultation and referral;
5. Partner with the peer leaders, participants and program facility host staff to plan and implement a
specially tailored program of classes, discussion groups, online virtual programming, and health
promotion activities aimed at enhancing mutual aid and the strengthening of informal social
support networks.
6. Maintain participant case records, progress notes, work logs and performance target outcome
tracking, and statistical reports.
Minimum Qualifications:
The position requires a bachelor’s degree in social work or related degree and experience in a social service setting.
Knowledge of and experience with older adult community-based services are also required.
Requirement:
As of 10/22/2021 all new hires at Neighborhood SHOPP must be vaccinated against the COVID-19 virus. If you are

submitted before your start date.
Neighborhood Self Help by Older Persons Project (SHOPP) is a non-profit social services agency founded in 1980
serving minority older adults throughout the Bronx. SHOPP’s mission is based on the philosophy of self-help. We
