OCES aims to support elders and individuals with disabilities by providing vital information and coordination of services that promote healthy, safe lives for our consumers. We want to help them stay as active and engaged in their lives and in their own communities for as long as possible.

To that end, OCES is excited to launch the new Enhanced Mental Health Outreach Team (EMHOT) and we are looking for a dedicated LICSW or LCSW to serve as the EMHOT clinician. The EMHOT clinician will use a person-centered, strength-based approach to work with individuals who have complex medical and behavioral health needs, both in short and long-term capacities.

What you'll do:

• Work directly with older adults with complex behavioral health conditions who require -term case management and/or psychotherapy to remain safely in the community.
• Conduct home visits as needed to assess and develop comprehensive, person-centered care plans with consumers, caregivers, and families.
• Provide advocacy, support, and assistance to address unmet needs including but not limited to housing, hoarding, substance use, medical case management, and communication with providers.
• Provide crisis intervention services, as needed.
• Conduct outreach to local providers about EMHOT services.
• Coordinate and/or facilitate support groups based on community need.

What you'll bring:

• Your passion for helping others and desire to make a difference
• Adaptability to be able to change as the needs of consumers and/or programs change
• Current LICSW or LCSW required
• At least three years of clinical experience preferably with older adults
