Job description

As a Clinical Population Health Assistant, you will audit physician charts and reach out to physicians to assist them in improving the use of Hierarchy Chronic Conditions (HCC). Research diagnostic codes and obtain consults or diagnostic results that were diagnosed by other physicians in the community to confirm the diagnosis is in the patients’ medical record.

Primary Duties and Responsibilities:

1. Works with physician to coordinate patient panel information available through Insurance company software to improve coding based on documented data.

2. Works with the TMPN management team and Medical Director to identify and develop education needed by physicians to meet RAF goals.

Assists with the education and support of Physicians and office managers regarding TMPN initiatives and processes to improve Quality and RAF initiatives.

Qualifications:

Bachelor’s degree or 2 years relevant clinical experience

Experience summarizing and presenting data and information to an audience such as physicians

Excellent interpersonal and communication skills

Highly proficient with Microsoft Office applications (Excel, Word, PowerPoint)

Job Type: Full-time

Pay: From $18.00 per hour
