Job Summary:
The Clinical Pharmacist is responsible for the provision of comprehensive clinical pharmacy services for Finger Lakes Community Health. This individual serves as a resource for the clinical care teams to determine the best medications and support in the delivery of medication therapy to patients with a broad range of disease states. This individual will be responsible for oversight and recommendations for health center medication formularies, storage, and administration. The pharmacist will apply comprehensive medication management principles to optimize outcomes in a patient- and family-centered approach.
Duties/Responsibilities:
Consultant:
- Complete chart reviews on patients to determine the appropriateness of medication therapy
- Collaborate with healthcare professionals and other healthcare providers to ensure optimal patient care
- Consult on pharmacotherapy: dosages, administration, duration, etc.
- Assess the results of pharmaceutical treatments
- Develop effective medication plans that minimize the risk of adverse side-effects
- Recognize untreated health problems that could be improved or resolved with appropriate medication therapy
Administration:
- Assess the status of the patient’s health status, disease state, and pharmacotherapy plan for optimization of the treatment plan.
- Evaluate the appropriateness and effectiveness of the patient’s medication plan
- Follow the patient’s progress to determine the effects of treatment
- Advise/educate the patient on the pharmacotherapy plan
- Educate the patient on methods to improve or maintain health, such as exercise, diet, and preventive steps (example: immunizations).
- Refer the patient to his or her physician or other health professionals to address specific health, wellness, or social services concerns
- Keep accurate documentation of medication plans and patient progress
Patient Care:
- Develop and maintain clinical pharmacy policies and procedures, including but not limited to proper handling and issuing of medication.
- Provide medication and practice-related education/training.
- Participation in the quality assurance program
- Participates/leads initiatives adopted by the Organization
Required Skills/Abilities:
- Possesses knowledge of healthcare privacy regulations
- Possesses strong organizational and time management skills
- Ability to communicate well written, orally, and frequently
Education and Experience:
- Maintain current NY Pharmacist license
- Board Certification Ambulatory Care (BCACP) is preferred
- Certification to administer immunizations in NY
Physical Requirements:
- Must be able to hear and communicate with clients and staff via phone and in person
- Must be able to lift up to 40 pounds.
- Must have vision that is adequate to read memo’s computer screen, registration forms, and other documents.
- Must possess manual dexterity, to perform writing and keyboarding tasks.
- Must be able to sit or stand for extended periods of time
- OSHA Level 1
