Title: Clinical Pharmacist
Reports to: Director of Clinical Pharmacy

Job Summary:
The Clinical Pharmacist position will be housed under a subsidiary organization of C3, Community Pharmacy Cooperative, LLC (CPC). CPC is focused on meeting the pharmacy-related needs of the ACO and other cooperative members.
This position is a unique opportunity for a Clinical Pharmacist to provide medication management expertise in an ACO environment. Through direct patient care, the Clinical Pharmacist will leverage their clinical expertise and knowledge of medications to ensure our patients have better health outcomes. The Clinical Pharmacist will work across departments to support Clinical Pharmacy Services as they relate to quality management, care management, and practice transformation. The Clinical Pharmacist position currently operates in a hybrid work model with many responsibilities achievable remotely.
- Collaborates with ACO and FQHC teams to provide and support Clinical Pharmacy Services across the Cooperatives
- Understands the interdependencies of C3/CPC pharmacy teams, FQHC Clinical Pharmacy teams and FQHC 340B pharmacy teams and works towards a seamless pharmacy experience for cooperative members
- Performs medication reconciliation creating the best possible medication lists for patients engaged in care management with the goal of reducing adverse drug events during transitions of care and optimizing future medication management
- May provide comprehensive medication review for high-risk populations (HF, COPD, etc.) with a goal of identifying drug related problems and optimizing guideline drive medication therapy
- May provide disease state management for specific populations based on organizational needs
- Educates patients on their medications and promotes effective medication use
- Identifies patient-specific barriers to adherence and develops plans to improve medication-taking behavior (ex. prepackaged medication packs, delivery, med synchronization, coordination with inhouse pharmacy)
- Assists in addressing escalated medication access issues related to affordability
- Communicates medication reconciliation, comprehensive medication review, or disease state management findings, recommendations, and interventions to ACO and FQHC care teams
- Documents encounters and interventions in care management and EMR platforms
- Represents the Clinical Pharmacy Team at internal and external meetings as assigned
- Acts as SME on drug information and medication management for other departments within the ACO when assigned
- Meets key performance indicators as determined by department leadership
- Other duties as assigned.

- Excellent written and verbal communication skills
- Experience caring for patients with complex, chronic medical and behavioral health needs
- Familiarity with the MassHealth ACO program, Medicare ACO program, and AQC is strongly preferred
- Familiarity with Federally Qualified Health Centers, Community Health Centers, or primary care practices strongly preferred
- Confidence with the technology used in communications, documentation, and record-keeping including Zoom, Microsoft Teams, Microsoft Word, Microsoft Excel
- Experience with EMRs and care management platforms including EPIC, Arcadia
- Experience with anti-racism activities, and/or lived experience with racism or training in or commitment to diversity, health equity, and social justice is highly preferred
- Bi-lingual in Spanish, Portuguese, Vietnamese, or Creole preferred
- Active, unrestricted MA pharmacist license (RPh)
- Bachelor's degree in Pharmacy or Doctor of Pharmacy (PharmD) degree from an ACPE accredited School of Pharmacy
- Valid driver's license and access to a working vehicle
- Completed ASHP Accredited PGY1 with Emphasis in Ambulatory Care or PGY 1 plus PGY 2 in Ambulatory Care preferred
- 3-5 years of clinical pharmacy experience preferred
- Board Certified or equivalent certification in area of expertise or willingness/ability to obtain within 2 years of employment preferred
- Light work: Exerting up to 20 pounds of force frequently to move objects. Many elements of the job are sedentary, but the employee may be required to stand for periods of time
