Overall responsibility as a leader to provide direct supervision of Clinical Staff (Homecare, Hospice, and Palliative Care staff) and oversight of all clinical patient care services in accordance with the Agency’s policies and procedures. The Clinical Manager reports to the Director of Homecare Operations.

EDUCATION AND EXPERIENCE

  • Graduate of a Baccalaureate program in Nursing or Therapy/Rehabilitation preferred.
  • Licensed as a Registered Nurse or Therapist (PT, OT, SLP) in Rhode Island and Massachusetts.
  • A minimum of five years of experience in Homecare (Home Healthcare, Hospice & Palliative and/or Rehabilitation).
  • Current certification in CPR.

QUALIFICATIONS

  • Basic knowledge of the practices of Home Healthcare, Hospice care and Rehabilitation.
  • Knowledge of community resources related to Home Healthcare, Hospice and Palliative care and Rehabilitation.
  • Knowledge of caseload requirements and what constitutes good caseload management and patient care.
  • Awareness and understanding of Agency admission policies for the Home Healthcare, Hospice and Rehabilitation programs.
  • Understanding and acceptance of Agency philosophy and the objectives and expectations of the agency programs.
  • Knowledge of the geographic area and communities the Agency serves.
  • Ability to set the climate for a productive work environment.
  • Knowledge of Agency’s personnel policies and union contracts.
  • Ability to maintain an overview of Agency functioning.
  • Ability to provide skilled and rehabilitative care based on scientific principles, basic behavioral concepts, and intelligent professional judgment.
  • Knowledge of Agency computer systems utilized for Home Healthcare, Hospice and Rehab programs.
  • Computer literate.
  • Demonstration of leadership skills.
  • Excellent interpersonal communication skills.
  • Demonstrates a positive attitude and willingness to support the Agency during times of change.

JOB SPECIFICATIONS

  • Full-Time, 40 Hours/Week / Salaried / Exempt
  • Weekends and Administrator On-Call rotation required depending upon agency needs
  • Portsmouth, RI Office

FEDERAL (MEDICARE AND MEDICAID) CONDITIONS OF PARTICIPATION

§484.105(c) Standard: Clinical manager.

One or more qualified individuals must provide oversight of all patient care services and personnel. Oversight must include the following:

  • Making patient and personnel assignments, G960
  • Coordinating patient care, G962
  • Coordinating referrals, G964
  • Assuring that patient needs are continually assessed and, G966
  • Assuring the development, implementation, and updates of the individualized plan of care. G968

§484.105(c) Interpretive Guidelines

§484.115(c) provides that a clinical manager must be a licensed physician, physical therapist, speech language pathologist, occupational therapist, audiologist, social worker, or a registered nurse.

PHYSICAL EFFORTS AND SKILLS

While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
