Full-time or Part time available!
Starting pay: $14.40/hr
7:45am-3:45pm Monday-Friday Work Week!

Job Requirements

Client Support Worker (CSW)

  • Valid Drivers License
  • Experience working with the Intellectually Developmentally Disabled population environment (Preferred)
  • Fingerprint Based Background Check of the Georgia and Federal Criminal History Record
  • Drug Screen
  • Motor Vehicle Report (MVR) for seven years
  • High school or equivalent (Required)

Job Description

  • Under direct supervision, assists with daily living activities at the person’s home, in a daytime non- residential facility or a residential facility. Provides training of basic personal and social skills, simple academics, or work skills to consumers/students in a state school, hospital, juvenile detention facility, residence, group home, workshop, or supported employment environment.
  • Provides services to individuals per ISP requirements using the person-centered approach.
  • Completes daily progress notes for assigned consumers.
  • Progress notes must match and reflect ISP recommendations.
  • Sample of charts indicate approved ISP's, HRST/ SIS reports, DMA7, annual updates on required forms, including Social, nursing and Psych assessments when completed and uploaded by the birthday and meeting minutes is completed and turned in reflecting any follow up that was needed.
  • At the time of the isp meeting, is prepared with meeting minutes, current isp review, hrst recommendations.
  • Completes the meeting minutes form (meeting section - follow up section due by birthday).
  • Complete and Upload HRST Monthly Data Trackers.
  • Completes daily goal tracking
  • Completing all agency trainings including Relias Training in timely manner as scheduled
  • Perform related duties as assigned by supervisor.
