QUALIFICATIONS:
- Graduate of a State Approved School of Nursing and have a current nursing license in the State of Indiana.
- Know all aspects of basic personal care, medication administration, treatment procedures, preventative, restorative and therapeutic techniques.
- Knowledge of medical asepsis and sterile technique.
- Interest in and be able to communicate effectively with aged and ill residents with variety of chronic illnesses or emotional disorders.
- Able to communicate effectively with visitors, supervisor and co-workers, and work under direct supervision of charge nurse.
- Able to maintain confidentiality of resident’s condition and personal affairs.
RESPONSIBILITIES:
1. Administer and Document Medications Given
a. Execute physician’s orders thoroughly and safely.
b. Administer and document all types of medications, according to pharmacy and drug administration policies and procedures.
c. Contact physicians and/or pharmacies, as necessary, for orders or prescription needs,
d. Receive telephone orders from physicians.
e. Use drug resource materials to ensure knowledge regarding action, dosage and side effects to drugs.
f. Ensure safe storage of medications.
g. Attend medication in services, as required by facility and State.
2. Perform Treatments and assist the physician.
a. Perform all treatments ordered by physician. In addition, perform naso-gastric and gavage feedings, insertion of straight and Foley catheters and naso-gastric tubes, administer special skin care procedures, sterile dressing changes, monitor IV fluid administration, oral-pharyngeal suctioning, protective devices, and special diagnostic tests.
b. Have working knowledge of emergency drug box policy and procedure.
c. Assist physicians with examinations and procedures.
d. Promote cost containment through careful and efficient use of supplies.
3. Observe, Record, and Report Observations
a. Monitor resident for expected and untoward responses from medication and therapeutic measures.
b. Conduct assessment and document when change in condition is noted using the Interact 3.0 system, SBAR and Care Pathways.
c. Notify Charge Nurse or Evening Shift Supervisor of any change in resident condition.
d. Give oral reports of resident condition to Charge Nurse and C.N.A.’s as required.
e. Contribute to completion of facility forms, such as Incident/accident forms, quality assurance audits, skin observations
f. Contributes to ongoing summary documentation and progress notes.
4. Assess Resident needs, Plan and Provide Resident Care
a. Assist with admission assessments and formulation of the plan of care.
b. Contribute information for revision in the resident plan of care.
c. Coordinate treatments with nursing assistants to provide comprehensive, effective and efficient resident care.
d. Plans and provides care which meets physical, psychological, spiritual, and rehabilitative needs.
e. Completes scheduled assessments on time and completely.
f. Uses opportunities for resident education in therapeutic and preventative health measures in collaboration with Charge Nurse.
5. Adhere to Safety and Infection Control Policies.
a. Practice good personal hygiene and adhere to policies and procedures.
b. Follow facility infection control policies and procedures.
c. Report defective equipment or other potential safety hazards promptly.
d. Initiate measures to protect residents from potential self-harm.
e. Attends in services required by facility and State.
6. Promotes Positive Public Relations for the Facility
a. Maintain attitude of courtesy, tact, and helpfulness when dealing with residents, visitors, physicians, consultants, therapists and other staff.
b. Use proper telephone etiquette.
c. Maintain confidentiality of resident condition and personal affairs.
7. Must supervise and work as a team player with the C.N.A.’s.
8. Follows all policies and procedures regarding the handling of controlled substances.
WORKING CONDITION:
- Works closely with residents who have a variety of chronic illnesses, or disabling and emotional conditions.
- Works with medical records.
- Operates special resident equipment for treatments; bathing, weighing and moving residents.
- Must be able to lift a minimum of 40 pounds.
- Walking, bending, stooping, moving, and transferring residents.
- Able to perform cardiopulmonary resuscitation (CPR) and be recertified every 2 years.
- Able to occasionally work overtime based on facility or residents needs.
