Job description
Work with incarcerated clients, providing direct and indirect services including but not limited to: Administer ASAM assessment; Develop individualized treatment and reentry/relapse prevention plans; Provide individual and group counseling/case management and crisis intervention. Assist with referrals and linkage to appropriate treatment and adjunct services in the community; Provide relapse prevention and independent living skills training, as appropriate
POSITION RESPONSIBILITIES:
Assess client’s level of psychosocial impairment, substance abuse history, motivation for change, self-care potential, family and support network availability, independent living needs, financial status, education, employment or training needs, and social/recreational needs, utilizing the ASAM assessment. Develop and coordinate service plans and make appropriate referrals post discharge.
Facilitate and provide SUD education and treatment in the form of individual and group counseling sessions and SUD brief interventions.
Assist clients in developing or maintaining the daily living skills needed to implement and achieve the goals of the treatment plan. Work towards gaining independent living skills and relapse prevention strategies to successfully integrate into the community.
Provide crisis intervention – intervene to include relapse prevention with the client at the onset of a crisis to provide support and assistance in problem resolution and to coordinate or arrange for the provision of other needed services.
Work as part of a multi-cultural, multi-discipline treatment team. Participate in Jail Health Services / SUD staff meetings, team case conferences; and other meetings as assigned.
Complete all SAPC documentation within mandated time frame and in compliance with county, state, and Center guidelines.
Meet daily program productivity standards.
Qualifications
Education:
Current, valid CCAPP/CADTP/CAADAC/CAADE certification- required
High School Diploma- required
Experience:
Minimum of two-years experience providing substance use disorder counseling in a mental health or substance abuse setting preferably with justice-involved, homeless, or dual diagnosed individuals. Experience in individual and group counseling- required
Job Type: Full-time
Pay: $48,000.00 - $50,000.00 per year
