Position Summary

JOB SUMMARY


This is entry level professional social service work in our County BehavioralHealth/Developmental Services Agency within the Early Intervention Unit. Employees who successfully complete a six month probation in this classification are promoted to the Caseworker classification.


Employees in this position participate in formal and informal BH/DS agency training programs which provide knowledge of the methods, procedures, rules and regulations necessary to perform social service and case management supportive services to families who have children with developmental disabilities or intellectual disabilities to assist them in attaining services/supports. The work assigned is limited in scope and difficulty, and is performed under close supervision, but as knowledge and skills are acquired, more latitude in judgment and decision making is permitted. Work also involves the training and application of problem solving techniques. Work is performed in accordance with established regulations, policies, and procedures, but employees are expected to exercise initiative and judgment in discharging their duties.


REPORTING RELATIONSHIPS


This position reports directly to their assigned supervisor. Work is reviewed by the assigned supervisor through regularly scheduled supervisions. Work is also reviewed through observations in the field, reviewing of records, provider and family contacts and others as identified by the supervisor as needed.


ESSENTIAL JOB FUNCTIONS


A support coordinator completes function in three areas of support coordination: locating, coordinating and monitoring.

  • Educating providers, families, and the community on Natural Environments and Early Intervention Philosophy
  • Informing families of appropriate community resources
  • Informing the family of the availability of advocacy services
  • Possessing effective communication and active listening skill
  • Understanding funding streams as they relate to Early Intervention
  • Coordinating the timely and accurate entry of data into LUIS, PELICAN, and/or EI Tracking databases
  • Developing, updating, and maintaining accurate paperwork in a timely manner.
  • Researching authorization issues as requested within two (2) business days of provider, EI management and/or fiscal department inquiries. Facilitating communication with service providers, Support Coordinators and EI management to bring issues to resolution and to facilitate timely payment of claims submitted.
  • Participating in the 24 hrs of required training imposed by OCDEL as well as other elective trainings as time permits
  • Maintaining knowledge of job responsibilities for Referral Support Coordinator and the processes of that job in order to complete the referral process timely
  • Assisting in the completion of bulk authorizations, at the start of the fiscal year, within three (3) business days for all services being received by children according to their Individualized Family Service Plan (IFSP)
  • Learning and monitoring the EI Tracking database for Early Childhood Outcome (ECO) data, Progress Monitoring Updates (PMUs), provider contract goals, and PELICAN system issues. Issuing appropriate reports to the providers monthly.
  • Reviewing status of PELICAN issues monthly and updating system as appropriate.
  • Assisting the Program Planner/Evaluator with provider monitoring
  • Serving as the PELICAN point person for the resolution of issues, changes and reporting tools
  • Serving as the ECO point person for the resolution of issues, changes and reporting tools

MINIMUM QUALIFICATIONS


A bachelor’s degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; OR

Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related socials sciences.

REQUIRED LICENSES/CERTIFICATIONS/CLEARANCES


Must pass pre-employment drug screening test.

Must have acceptable background check that includes FBI Clearance, PA State Police criminal history and ChildLine clearances. Clearances are required prior to the start of employment and may not be more than one year old at time of employment.

A valid driver’s license and acceptable driving history in accordance with County policy.
