CASE MANAGER
Position Description:
Under the supervision of the Clinical Supervisor, the Case Manager performs a variety of clinical and administrative work involving the delivery of mental health services to adults in need of treatment and support. The incumbent will work with a team of clinicians in the planning, implementation and performance of therapeutically oriented activities and programs for consumers receiving mental health and substance abuse treatment services. The incumbent must possess considerable knowledge of mental health disorders and treatment methods, as well as an understanding of the principles and techniques used in the treatment of mental health disorders.
Duties and Responsibilities:
Clinical
1. Maintain a caseload of individuals with substance abuse, social behavior, health, and living issues
2. Provide services to client in multiple settings including both facility and home environment
3. Assist clients in adhering to treatment plans, such as setting up appointments, arranging for transportation to appointments, or providing clinical support.
4. Educate clients, family members, and/or guardians about mental or physical illness, abuse, medication and have ability to provide community resources.
5. Refer client and family to community resources for housing or treatment to assist in recovery from mental or physical illness and follow through to ensure service efficacy.
6. Communicate with team therapist the goals, objectives, and interventions that should be updated on treatment plan for shared client
7. Observe and report behavior, attitude, mood swings, mental health and/or healthcare, living needs.
8. Ensures compliance with legal issues including but not limited to client confidentiality and risk management; ensures compliance with federal, state, and local regulations
Administrative
1. Manage own schedule with at least two visits per week.
2. Attend bi-weekly staff meeting, required trainings and/or supervision, and appraisal and disciplinary sessions
3. Completion of self-evaluation at 90 day and annual employment mark
4. Document clinical sessions in BIRP format (Behavior, Intervention, Response, Plan)
5. Adhere to organizations policies and procedures
Minimum Qualifications:
1. Must have a Bachelors’ Degree in clinical psychology, counseling or a related field is preferred.
2. Possess at least 1-2 years’ experience with providing Mental health services
3. Knowledge of mental health disorders and treatment methods, preferably in a behavioral healthcare setting.
4. Ability to provide skilled treatment to clients with social and behavioral problems.
5. Ability to establish and maintain working relationships with consumers, families, service providers and colleagues.
6. Exceptional verbal and written communication skills.
Job Types: Full-time, Part-time
Pay: $15.00 - $33.00 per hour
