Job Summary and Qualifications

Provides ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. Enhances the quality of patient management and satisfaction to promote continuity of care and cost effectiveness through the integration of case management, utilization review management and discharge planning.

Schedule: Full-time, Days (with weekend rotation)

What qualifications you will need:

• Associate’s degree or diploma in Nursing required
• Florida Registered Nurse (RN) license required; or appropriate multi-state license
• Minimum of three (3) years recent acute clinical practice or related health care experience required

Skills Needed:

Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills, Demonstrates analytical and critical thinking abilities with pro-active decision-making and negotiation skills, strong knowledge of governmental and private organization and community resources.

This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).

HCA Florida Lake Monroe Hospital is a 230-bed acute care hospital. We serve the communities of Seminole and West Volusia County. At HCA Healthcare, we are committed to the care and improvement of human life. We are a Level II Trauma Center. We provide the only full-service cardiovascular program. Our program offers open-heart surgery and interventional cardiology. Electrophysiology, cardiac rehabilitation, and comprehensive diagnostic services are also offered by this program. Other services that we provide include orthopedic and spine care, as well as robotic surgery. We offer acute inpatient medical rehabilitation and complete imaging services.
