Catholic Social Services Permanent Supportive Housing (PSH) program provides supportive services to individual and families who have experienced homelessness. The main focus of the PSH program is to help individuals and families identify barriers to housing stability and engage in goal planning that link individuals to resources and skills in order to achieve long-term housing stability. Additionally, Catholic Social Services connects families to resources that are available within their respective communities to sustain long-term stable housing.
The Case Manager is responsible for working closely with the Program Coordinator and other employees to ensure a positive and safe environment for clients while ensuring the functions of the program are met. Completes initial needs assessments of clients and acts upon critical needs appropriately and immediately while developing a comprehensive, client driven, case plan with both short-term and long-term goals.
The Case Manager is also responsible for ensuring the functions of the program are met while providing services that are safe, timely, effective, efficient, equitable and client-centered.
Qualifications include:
- Bachelor’s degree in social work, mental health counseling, or related field preferred
- Experience working with clients with complex presentations including chronic trauma
- Must be comfortable working in community settings
- High level of personal accountability for the quality and impact of work
- Reliable transportation required
- Bi-lingual helpful
